Applicants for permits shall be made to the City Controller's Office on the forms providing for the particular permit and shall include the name, address and telephone number of the applicant, and type of permit applied for. Permits are to be issued for a term of 1 year except where state statutes, laws, regulations require otherwise. All fees shall be paid at the time of application and are not refundable.
