Upon approval by the Taxicab Inspector, there shall be issued on a form prepared by the Inspector and Chief of Police, a taxicab driver’s permit which shall contain a photograph of the driver, his or her right thumbprint, permit number, date issued, expiration date, name, age, weight, height, race, color of hair and eyes, employer and signature of the driver. Said driver’s permit shall be signed by the Chief of Police or the Inspector of Taxicabs.
