§ 5.02 WRITTEN NOTICE.
   When a licensee fails to respond to the informal notice, a written notice shall be given. This written notice shall be dated and shall call attention to specific items of concern. The notice shall be either hand delivered to the licensee or mailed to the licensee at its address as shown on its license. The procedure set forth in this chapter shall be clearly printed on the back of such notice.
