(A) Application for a license to collect garbage and/or rubbish and/or other refuse within the city and/or to transport garbage and/or rubbish and/or other refuse over the streets of the city shall be made at the office of the City Clerk on forms provided by said office. With his or her application, the applicant shall file a certificate or affidavit of insurance as hereinafter set forth and shall deposit the required license fee.
(B) Upon receipt of such application properly executed by the applicant, the City Clerk shall refer same to the Street Superintendent for his or her review and approval.
