§ 112.05 APPLICATIONS FOR CITY ALCOHOLIC BEVERAGE LICENSES.
   Applications for the city alcoholic beverage licenses required by this chapter shall be on a form provided by the Administrator, which shall include all of the information required in the application for a corresponding state alcoholic beverage license, in addition to any other information required by the Administrator. The application shall be signed at the end by the applicant on a line immediately following a declaration that false statements in the application shall constitute the crime of perjury; and the signature of the applicant shall then be notarized according to law. The application shall be submitted to the Administrator, along with the fee established herein for the alcoholic beverage license that is the subject of the application, which shall be paid to the city by cash, bank cashier’s check or credit card.
