7-2-5.3: APPLICATIONS:
Any person desiring to operate a food establishment, retail food store, mobile unit, temporary or seasonal facility, or farmers' market shall make written application for a permit on forms provided by the health authority. Such application shall include at minimum the applicant's full name, mailing address, phone number, e-mail address, and whether such applicant is an individual, firm, corporation, or partnership; as well as the location of the proposed facility, and the signature of the applicant. More information may be required by the health authority as deemed necessary and the information provided on the application shall be kept current at all times. Applications for temporary or seasonal permits shall also include the inclusive dates of the proposed operation and a menu. Application for plan review shall be made for any operation requiring new construction, remodeling, or a change of use prior to being approved.
