3-23-1: REGISTRATION AND FEES:
   (A)   Any septic waste hauler wanting to obtain dumping privileges at the East Moline Regional Wastewater Treatment Plant will be required to register and provide the following information:
      1.   Billing information (W-9 form requires as part of billing information);
      2.   Emergency contact information;
      3.   Proof of liability insurance (Min. $100,000.00 property / $300,000.00 personal injury);
      4.   Proof of worker’s compensation insurance (or sworn statement applicant has no employees);
      5.   List of trucks with tank volumes;
      6.   List of drivers with relevant information;
      7.   List of other disposal sites used by waste hauler.
   (B)   All septic waste haulers are required to abide by the City of East Moline’s Sewer Use Ordinance, East Moline’s Dumper Policy, US EPA and IEPA requirements pertaining to septic waste haulers.
   (C)   All septic waste haulers will be required to pay a $50.00 non-refundable application fee that will accompany the registration application.
   (D)   If approved, the registration will be valid until January 31 of the following calendar year at which time the registration must be renewed. (Ord. 18-13, 10-15-2018)
