An application for a license for any of the sales regulated in this chapter shall be made at the Office of Planning and Development, on a form to be provided therefor and shall contain the following information:
(a) The name and permanent address of the person, firm, group, corporation, association or organization conducting the sale;
(b) The name and address of all persons who will be associated or engaged in such public sale if the sale is to be a joint sale or joint venture. Each person in a joint venture will be considered to have been issued a license for that venture, as applicable to Section 785.04
;
(c) The name and address of the party for whom the sale is being held, if different from the applicants'.
(d) The location at which the sale is to be conducted;
(e) The number of days which the sale is to be conducted with the limit hereinafter described;
(f) The date and nature of any past sale held by the applicant subject to any regulation of this chapter;
(g) Whether or not the applicant has been issued a vendor's license by the State and if so, the number of such license and the date on which the same was issued;
The application may be made by mail. The license fee shall be five dollars ($5.00) for each separate sale period or ten dollars ($10.00) for maximum nine-day multi-period sale.
