Application for the license required by this chapter shall be made in writing upon blanks furnished by the Mayor, and the application shall contain:
(a) The full name of the applicant;
(b) His address;
(c) The name of his former or present employer, if any;
(d) The names of two persons who may be referred to as to such applicant's character;
(e) The number of tables and alleys to be so used;
(f) The location and a description of the room or place to be so occupied;
(g) The applicant's own statement as to whether or not he has ever been convicted of any offense under the laws of the United States, or of this State or the ordinances of the City relating to traffic in intoxicating liquors, and shall be signed by or for such applicant.
Such application shall be filed with the Mayor.
