The application for the license required by Section 749.03, or an accompanying affidavit, shall be in writing and shall contain the following facts:
(a) Reasons for, and character of, the temporary business which the applicant desires to conduct.
(b) Names and addresses of persons from whom any goods, wares or merchandise, which are intended for sale, were purchased.
(c) Name and address of the owner or person in whose interest the business is being conducted.
(d) Places and dates where the applicant has been engaged in business for the past eighteen months, together with the nature and character of such business.
(e) All details necessary to exactly locate and fully identify all goods, wares and merchandise to be sold.
(f) The length of time for which the store or place where such goods, wares or merchandise are to be sold, has been leased or is to be leased.
Such facts as are set forth in the application or an accompanying affidavit, shall be sworn to under oath before an officer empowered by the laws of this State to take acknowledgment of deeds.
