(A) The application for a license shall contain the following information:
(1) Name and address of the applicant, age, date and place of birth;
(2) Prior convictions of applicant, if any;
(3) Place where machine or device is to be displayed or operated and business conducted there; and/or
(4) Description of machine or device to be covered by the license, mechanical features, name of manufacturer and serial number.
(B) No license shall be issued to any applicant unless he or she shall be over 21 years of age and a citizen of the United States.
