14-3-3: REGISTRATION REQUIREMENTS:
For each abandoned structure each owner shall register with the city and provide the following information on an abandoned/vacant structure registration form available from the city:
   A.   The address of the abandoned structure;
   B.   The legal names of all owners of the property and each owner's date of birth, if available;
   C.   The complete mailing address of all owners;
   D.   Telephone numbers of each owner, including cellphone and mobile phone numbers;
   E.   Proof of identification of each owner; (Ord. 13-0213, 2-19-2013)
   F.   The name, address and telephone number of any local agent or representative authorized by the owner to handle the affairs of the property. If the property is marketed for sale, it will be exempt from these provisions; (Ord. 15-0205, 2-2-2015)
   G.   The reason for vacancy of the property;
   H.   The estimated length of time the property is expected to remain vacant;
   I.   Any plans for restoration, reuse or removal with an accompanying time line and work schedule; (Ord. 13-0213, 2-19-2013)
   J.   Proof of insurance (insurance document with the name of insurance company, policy number, and effective dates). (Ord. 15-0205, 2-2-2015)
An owner shall notify the city and file an amended form within seven (7) days of any change in the registration information required by this chapter. (Ord. 13-0213, 2-19-2013)
