An application for a game room license and for amusement device permits shall be made to the Mayor upon forms to be prescribed by the Mayor. The applicant shall attach to the application a floor plan, drawn to scale, by a qualified engineer, architect or draftsman, showing the proposed placement of each amusement device on the applicant’s premises. All regular and emergency exits and all other pertinent equipment shall be clearly identified on the floor plan.
