(A) Information required. An applicant for a business license issued under this chapter shall complete an application form prepared and made available by the City Controller and upon presentment and surrender of the receipt of the City Controller showing payment of the license fee as provided in § 110.060. Said application shall contain or state the following information:
(1) The full legal name, age, residence, and address of the applicant and business owners.
(2) Whether the applicant is an incorporated firm, association, or corporation.
(3) If applicant is an incorporated firm or partnership, the application shall contain the full name and residence addresses of every individual composing such firm or partnership, including full names, residential addresses, and telephone numbers of registered agents.
(4) All applicable federal and state identification numbers.
(5) If applicant is a limited partnership, the full names and residence addresses of the general partners thereof.
(6) If applicant is a corporation, the full names and residence addresses of its officers and resident agents and the place where and the time when it was incorporated.
(7) If applicant is a corporation, a copy of its current official Certificate of Existence or Authorization issued by the State Secretary of State, or other state in which the corporation is registered.
(8) The exact location of the place of business or proposed location for which the license is sought.
(9) The nature of the business, occupation, thing, or device for which the license is sought.
(10) Whether applicant is a citizen of the United States or resident alien.
(11) The experience, if any, the applicant has had in such business.
(12) The designation of a mailing address which will be acceptable for the receipt of letters or notifications to appear before the proper city authorities to show cause why such license should not be revoked for failure to comply with the code or other ordinance of the city. For the purposes of this section, a post office box does not suffice as an address.
(13) The hours of operation for the proposed retail or service establishment.
(14) Any other information the City Controller shall find reasonably necessary to effectuate the purpose of this chapter, and to arrive at a fair determination of whether the terms of this chapter have been complied with.
(B) Licenses; fees and renewal.
(1) An application for renewal shall be first approved and certified by the city controller before a renewal is granted and issued.
(2) The annual business license renewal fee for a business in good standing shall be determined according to the current Business License Schedule of Fees.
(3) The full annual fee must be paid in full before a license or renewal is issued, and there shall be no prorating of fees.
