(A) The city shall supply free of charge diphtheria, scarlet fever, tetanus (lockjaw) antitoxin, and antirabic virus to people too poor to purchase the same, when proper blank forms are filled out in compliance with this section and other law.
(B) Upon the receipt of an official blank duly executed, any dealer may supply the antitoxin or virus, and the blank shall be a legal claim against the city for the market price of the antitoxin or virus.
(C) All costs hereby incurred through legal purchases of antitoxin or virus shall be paid by the city.
(D) The City Health Officer shall notify the Prosecuting Attorney of any violation of this section and shall also make reports required by law.
