§ 51.088 PERMIT CONDITIONS REQUIRED.
   Wastewater discharge permits shall specify no less than the following:
   (A)   Fees and charges to be paid upon the initial permit issuance;
   (B)   An identification number which correlates to the discharge location being regulated by the permit;
   (C)   Permit issuance, effective and expiration dates;
   (D)   Effluent limitations and expiration dates;
   (E)   Requirement to control slug discharge in accordance with 40 C.F.R. § 403.8(f)(iii)(1)(B)(6) if determined to be necessary by the District;
   (F)   Record keeping requirements in accordance with 40 C.F.R. § 403.8(f)(iii)(1)(B)(4);
   (G)   For those IUs requiring a pretreatment plant or construction or installation of facilities or equipment to process wastewater prior to discharge to the District, the requirement for such facilities to be under the control of a qualified operator certified pursuant to the provision of 327 I.A.C. § 5-22-1, et seq.;
   (H)   Special conditions as the District may reasonably require under particular circumstances of a given discharge including sampling locations, frequency of sampling, number, types, and standards for tests and reporting schedules;
   (I)   Requirements for installation and maintenance of inspection and sampling facilities and equipment, including flow measurement devices;
   (J)   A statement that compliance with the individual wastewater discharge permit does not relieve the permittee of responsibility for compliance with all applicable federal and state pretreatment standards, including those which become effective during the term of the individual wastewater discharge permit; and
   (K)   Compliance schedules, if necessary;
   (L)   Submission of special technical reports or discharge report where same differ from those prescribed by this chapter when requested by the District;
   (M)   Statement of permit duration pursuant to § 51.090;
   (N)   Statement of permit non-transferability pursuant to § 51.091;
   (O)   Statement of applicable enforcement actions pursuant to §§ 51.135 through 51.147; and
   (P)   Statement of applicable penalties for non-compliance pursuant to § 51.999.
