56.01   ANNUAL LICENSE REQUIRED.
   1.   Every owner of a dog over the age of four months must procure a dog license from the Clerk on or before the first day of January of each year.
   2.   Such license may be procured after January 1 and at any time for a dog which has come into the possession or ownership of the applicant or which has reached the age of three months after said date.
   3.   The owner of a dog for which a license is required must apply to the Clerk on forms provided by the Clerk.
   4.   The form of the application must state the breed, sex, age, color, markings, and name, if any, of the dog, the address of the owner, and shall be signed by the owner. The application must also state the date of the most recent rabies vaccination, the type of vaccine administered, and the date the dog must be revaccinated.
   5.   All licenses will expire on January 1 of the year following the date of issuance.
