Each final plat submitted to the Commission for approval shall carry a certificate signed by a Registered Professional Surveyor or Registered Professional Engineer, in substantially the following form:
“I, , hereby certify that I am a Registered Professional Land Surveyor or Registered Professional Engineer of the State of Indiana; that this plat correctly represents a survey completed by me on , 19 , that all the monuments shown thereon actually exist, and that their location, size, type and material are accurately shown, and comply with the provisions of the Subdivision Control Ordinance.” (Ord.
