§ 97.064 CONTENTS OF NOTICE.

   Every notice given by an excavator to the city pursuant to this section shall contain at least the following information:

   (A)   The name of the individual and or company serving such notice;

   (B)   The location of each proposed area of excavation or demolition;

   (C)   The name, address, and telephone number of all excavators or excavator’s companies;

   (D)   The excavator’s field telephone number;

   (E)   The type and extent of the proposed work;

   (F)   Whether or not the discharging of explosives is anticipated; and

   (G)   The date and time when work is to begin and end.
