§ 111.13 FORM AND CONTENTS OF LICENSE.

   A business license issued pursuant to this chapter shall show:

   (A)   The name of the person licensed;

   (B)   The place where the business is conducted (if it is to be conducted at one place);

   (C)   The nature of the business licensed;

   (D)   The period for which the license is issued; and

   (E)   The amount of the license fee paid.
