The statement of financial interest shall be filed on a form prescribed by the Board of Ethics or the administrative official designated by the Board of Ethics. The Board, or the designated administrative official, shall deliver a copy of the form to each officer, employee or filer required to file the statement, by first class mail or hand delivery, no later than the 15th day of December of each calendar-year. The failure of the Board, or the designated administrative official, to deliver a copy of the form to any official, officer, employee or filer, shall not relieve the official, officer, employee or filer of the obligation to file the statement.
