715.02 CERTIFICATE OF PUBLIC CONVENIENCE AND NECESSITY REQUIRED.
   An application for a certificate shall be filed with the Mayor upon forms provided by the Village; and such application shall be verified under oath and shall furnish the following information:
   (a)   The name and address of the applicant.
   (b)   The experience of the applicant in the transportation of passengers and the names and addresses of all municipalities in which the applicant is licensed.
   (c)   The number of vehicles to be operated or controlled by the applicant and the location of proposed depots and terminals.
   (d)   The color scheme or insignia to be used to designate the vehicle of the applicant.
   (e)   The names, addresses, Social Security number and driver's license number of all drivers.
   (f)   Such further information as the Mayor may require.
