721.03 APPLICATION FOR LICENSE.

   Application for the license shall be made to the Mayor upon blanks to be furnished by the Mayor for that purpose, containing the following information:

   (a)    A brief description of the coin-operated device to be registered, including the name of the manufacturer, the factory number of the device and the year's model.

   (b)    The name, residence and business address of the owner and the township, city or village in which the owner resides.

   (c)   The place in the City where the coin-operated device is to be operated.

   (d)    Whether the coin-operated device is new or used.

   (e)    Date of purchase of the coin-operated device.

   (f)   Whether the fees to be paid for the registration of the coin-operated device during the preceding registration year, if any, have been paid.

   (g)    The nature of the business conducted at the place in the City where the coin- operated device is to be operated.

   (h)    Such other and further information as the Mayor may require.

   Each application for registration shall be signed by the owner of the coin-operated device or his or its authorized agent, before the Mayor. (Ord. 1996. Passed 5-2-55.)
