(A) A written application for a permit to solicit for any cause as provided in § 7-3-1, shall be sworn to and filed with the Chief of Police, and the application shall contain the following information:
(1) Name and address of organization or individual applying for a permit to solicit;
(2) Names and addresses of principal officers and management if applicant is other than an individual;
(3) The purpose for which any receipts derived from the solicitation are to be used;
(4) The name and address of the person by whom the receipts of the solicitation shall be disbursed;
(5) The name and address of the person who will be in direct charge of conducting the solicitation;
(6) An outline of the methods to be used in conducting the solicitation;
(7) The time, giving the proposed beginning and ending dates when the solicitation will be made;
(8) The amount of any wages, fee, commissions, expenses or emoluments to be expended or paid to anyone in connection with the solicitation and how expended or to whom paid;
(9) A financial statement for the last preceding year of any funds collected for the purpose given above, the statement to show the amount collected, cost of collection and the final distribution of the funds raised;
(10) A full statement of the character and extent of the charitable, educational or philanthropic work being done in the city by the applicant;
(11) Other information as may be required to fully determine the kind, character and worthiness of the proposed solicitation;
(B) The permit application shall be accompanied by such fee as fixed from time to time for itinerant businesses.
