§ 155.05  IMPACT STATEMENT.
   Any person, firm, or corporation, or other entity, who shall apply, or who is required to apply, for a permit to drill a well within the zone of protection of the village shall supply to the village an impact statement. The impact statement shall be prepared by a registered professional engineer from the state, licensed by the appropriate licensing authority for registered and professional engineers in the state. The impact statement shall show, in detail, the type of well construction including, but not limited to, the depth, the proposed formation into which he or she expects to drill, the amount of water he or she expects to pump per day, whether the well will be cased or uncased, how he or she intends to use the water, the amount of water he or she expects to pump daily, weekly, monthly, and annually, the proposed method of sealing between separate water bearing formations, the expected annual draw down depth from the top of the aquifer into which the well is placed and what, if any, resulted impact this would have on the village’s water supply, and whether or not the use contemplated is a reasonable use.
