An applicant desiring to secure such license shall file with the Village Clerk his or her application in writing, signed by him or her, stating the name and address of the owner-operator, the number of coin-operated devices which he or she has in operation in the village, and/or the number of billiard and pool tables which he or she has in operation in the village, the address of which each such device, billiard, or pool table is located, and the type of business in which it is operated. Amendments to this application shall be filed whenever new locations or new machines, billiard, or pool tables are added. Upon payment of fees, the Village Clerk shall then issue said license. The license shall be non-assignable and non-transferable.
