(A) A non-refundable fee of $125 shall be paid at the time of the submittal of an initial or renewal application for a sexually oriented business employee license. A minimum non-refundable fee of $250 shall be paid at the time of the submittal of an initial or renewal application for a sexually oriented business.
(B) (1) Each application shall contain a provision on the application in bold print stating that any withholding of information or the providing of false or misleading information will be grounds for the denial or revocation of a permit. Any changes in the information provided on the application or provided during the investigation shall be reported in writing to the city by the owner(s) or applicant(s) or permittee within ten days of change. If said changes take place during the investigation, the data shall be provided to the enforcement officer in writing.
(2) Failure to report said changes by the applicant(s) or owner(s) or the permittee within ten days may result in a denial or revocation of a permit.
(C) (1) The enforcement officer shall cause criminal background checks to be performed on each owner, applicant and licensee as well as 5% or more owners of a corporation, partnership, LLC or other business entity who is required to sign an application. Costs of up to four criminal background checks are included as part of the permit fee for a sexually oriented business license.
(2) In the event more than four criminal background checks are required, an additional charge of $30 shall be charged for each additional criminal background check required.
