Pursuant to the authority granted in I.C. 16-20-1-27, the Board of the Health Department, with the approval of Council, establishes and collects the following fees.
(A) Vital records fees.
(1) Birth certificate $15
(2) Death certificate, long form $30
(3) Death certificate, short form $20
(4) Paternity affidavit $50
(5) Birth certificate amendment $25
(6) Exceptions. Pursuant to I.C. 16-37-1-9(c), a charge may not be made for furnishing a certificate of birth, death, or stillbirth registration to a person or to a member of the family of a person who needs the certificate for one of the following purposes:
(a) To establish the person’s age or the dependency of a member of the person’s family in connection with:
1. The person’s service in the armed forces of the United States; or
2. A death pension or disability pension of a person who is serving or has served in the armed forces of the United States.
(b) Pursuant to I.C. 16-37-1-9(d), a charge may not be made for furnishing a certificate of birth to an individual who represents, pursuant to I.C. 31-36-3-4(b), a homeless youth (as defined in I.C. 31-36-3-4) and presents a fee and consent waiver affidavit described in I.C. 31-36-3-4(c).
(B) Immunization fees
(1) Establishment of fees.
(a) The County Department of Health hereby establishes fees for public health services as described herein.
(b) All charges for services/tests will be assessed to clients depending on applicability as follows:
1. At the true cost to provide the service; or
2. At the current Federal Medicaid/Medicare or comparable reimbursement rate; or
3. A lesser value determined to be reasonable. In no event will the fees be greater than the actual cost to provide the service/test.
(c) The price policy for immunizations will be posted and the collected fees will revert into the Health Fund.
(2) Vaccines available for private purchase. Adults over 18 years of age who request the following vaccines:
(a) Td $25
(b) Tdap $35
(c) Hepatitis B $40/dose/three-dose series
(d) Hepatitis A $35/dose/two-dose series
(e) IPV $30
(f) Menactra $110
(g) Pneumococcal (PPSV23) $65
(h) Prevnar13 $155
(i) Flu $10
(j) Shingles $200
(k) MMR $60
(l) Typhoid $60
(m) Varicella $95
(3) TB test. The current cost of a TB test is $10.
(C) Onsite sewage disposal fees.
(1) Definitions. For the purpose of this division, the following definitions shall apply unless the context clearly indicates or requires a different meaning.
ANNUAL HOLDING TANK OPERATING PERMIT. An annual permit for maintaining and operating a wastewater holding tank to be used solely for the purpose of maintaining wastewater in a sanitary manner and is on a service contract with a registered service provider.
ANNUAL ONSITE SYSTEM OPERATING PERMITS. An annual operating permit for onsite
systems requiring a maintenance contract.
COMMERCIAL ONSITE SEWAGE DISPOSAL SYSTEM or ONSITE SEWAGE DISPOSAL SYSTEM (OSDS). All equipment and devices necessary for proper conduction, collection, storage, treatment, and onsite disposal of sewage from other than one-family or two-family dwellings, except where such dwellings are connected to a cluster system. However, an onsite sewage system serving two single-family dwellings on the same property, with a combined daily design flow (DDF) of less than or equal to 750 gallons per day, is a residential onsite sewage system, not a commercial onsite sewage system. Included within, but not limited to, the scope of this definition is building sewers, grease traps, septic tanks, dosing tanks, absorption fields, perimeter drains, vault privies, and temporary sewage holding tanks serving commercial facilities.
COMMERCIAL ONSITE SEWAGE SYSTEM PERMIT. A permit for the installation of a commercial onsite sewage system.
COMMERCIAL ONSITE SEWAGE SYSTEM REPAIR PERMIT. A permit for the installation of a commercial onsite sewage system repair.
COMMERCIAL ONSITE SYSTEM REPLACEMENT PERMIT. A permit for the installation of a commercial onsite sewage system replacement.
DEPARTMENT. The Dearborn County Health Department.
DISPOSAL CONNECTION APPLICATION FEE. The fee for department staff to verify arrangements for a sanitary sewer connection has been secured by the applicant as per BC Ordinance 3-1997.
EXISTING ONSITE SYSTEM PERFORMANCE REVIEW. The review of an existing onsite sewage disposal system (OSDS) at the request of the applicant, for its ability to meet the Rules and Regulations of Indiana and to comply with the requirements of a regional sewer district or similar agency.
INITIAL SITE VISIT FOR ONSITE SYSTEM INSTALLATION REVIEW. The initial site visit request by an applicant for the installation of a new, repair, or replacement onsite sewage disposal system. This term shall also apply for the initial site visit request for verification that a proposed outbuilding or residential or commercial expansion location does not interfere with the existing onsite system.
ONSITE SEWAGE DISPOSAL INSTALLER. Any person engaged in the construction, installation, repair, or replacement of any component of an onsite sewage disposal system (OSDS).
ONSITE SEWAGE DISPOSAL SYSTEM (OSDS) REPAIR. The repair or replacement of any OSDS component other than the replacement or expansion of a soil absorption field.
ONSITE SEWAGE DISPOSAL SYSTEM (OSDS) REPLACEMENT. The replacement or expansion of a soil absorption field.
ONSITE SYSTEM PERMIT APPLICATION REVIEW. An application filed with all pertinent
information for onsite system permit issuance review.
REGISTERED ONSITE SEWAGE DISPOSAL INSTALLER. An onsite sewage disposal installer who has completed an application and has passed a written proficiency test as administered by the department.
REGISTERED ONSITE SEWAGE DISPOSAL INSTALLER ANNUAL FEE. The annual fee for a registered onsite sewage disposal installer starting on January 1 of the year of registration and ending on December 3l of the year of registration.
RESIDENTIAL ONSITE SEWAGE DISPOSAL SYSTEM or ONSITE SEWAGE DISPOSAL SYSTEM (OSDS). All equipment and devices necessary for proper conduction, collection, storage, treatment, and onsite disposal of sewage from:
1. A one or two family dwelling;
2. A residential outbuilding; or
3. Two single-family dwellings on the same property with a combined daily design flow (DDF) of less than or equal to 750 gallons per day.
The term includes, but is not limited to, residential sewers, septic tanks, soil absorption systems, temporary sewage holding tanks, and sanitary vault privies.
RESIDENTIAL ONSITE SEWAGE SYSTEM PERMIT. A permit for the installation of a residential onsite sewage system.
RESIDENTIAL ONSITE SEWAGE SYSTEM REPAIR PERMIT. A permit for the installation of a residential onsite sewage system repair.
RESIDENTIAL ONSITE SEWAGE SYSTEM REPLACEMENT PERMIT. A permit for the installation of a residential onsite sewage system replacement.
(2) Fees.
