A licensed secondhand goods dealer or pawnbroker must provide a receipt to the seller or consignor of any item which includes:
(A) The address and telephone number of the business;
(B) The date;
(C) A description of the items purchased; and
(D) The purchaser’s or co-signee’s signature.
