§ 50.073 PERMIT APPLICATION.

   (A)   The application required under this chapter shall be on a form specified by the Judge/Executive or his or her designee.

   (B)   Each applicant for any such permit shall state in his or her application the following:

      (1)   The nature of the permit desired, whether:

         (a)   To collect;

         (b)   To process; or

         (c)   To transport solid waste or any combination thereof.

      (2)   The name and address of the applicant and whether a sole proprietorship, corporation, partnership, limited liability company, or other entity, with disclosure of the ownership interests;

      (3)   The number of employees and solid waste vehicles to be operated thereunder, together with a description showing that each such vehicle meets the requirements of this chapter;

      (4)   The location or locations of solid waste disposal facilities or solid waste processing facilities to be used, including the facility name, permit number, anticipated volume, and types of solid waste to be disposed or processed; and

      (5)    A certification. The application shall include a certification as follows.

 

I hereby certify that all of the information contained in this application is true and accurate. I understand that penalties for violation of this permit may include fines of $25 to $100 per day, per occurrence, and injunctive relief.
