§ 32.34 CONTENTS OF THE FINANCIAL INTEREST STATEMENTS.
   (A)   The statement shall include the following information for the preceding calendar year:
      (1)   The name, current business address, business telephone number and home address of the filer;
      (2)   The title of the filer’s office, office sought or position of employment; and
      (3)   The occupation of the filer and filer’s spouse.
   (B)   Except for the individuals listed in § 32.30(C), the filer shall disclose the following information as it applies to the filer and the filer’s dependent family:
      (1)   Each source of income exceeding $5,000 during the preceding calendar year, and the nature of the income (e.g., salary, commission, dividends, rents, etc.).
      (2)   The name and address of any business in which at any time during the preceding calendar year there was an interest of $10,000, at fair market value of 5% ownership interest or more.
      (3)   Each source by name and address of any creditor for which there is a personal liability of more than $10,000, except debts arising from the purchase of consumer goods or a primary residence.
      (4)   Each source, by name and address, of gifts or honoraria having an aggregate fair market value of $200 or more from any single source, excluding gifts received from family members during the preceding calendar year.
   (C)   The individuals in § 32.30(C), and only those in § 32.30(C) shall list the location of all real property within the county, other than the filer’s primary residence, in which the filer or the filer’s dependent family member had during the preceding calendar year an interest of $10,000 or more, and designate such property as commercial, residential, or agricultural.
   (D)   Nothing in this chapter shall require the disclosure of any specific dollar amounts nor the names of individual clients or customers of businesses listed as sources of income.
   (E)   Each statement shall be signed and dated by the filer.
