(A) No person shall perform a tattooing procedure on an individual who is under 16 years of age, with or without parental consent, and no person shall perform a body-piercing procedure on an individual who is under 14 years of age, with or without parental consent. Parental consent is required to perform a tattooing procedure on any individual between 16 and 18 years of age, and parental consent is required to perform a body-piercing procedure on any individual between 14 and 18 years of age. Tattooing or body-piercing may be performed on individuals 18 years or older, without parental consent.
(B) A parent, guardian or custodian of an individual under age 18, but over 16 years of age, who desires to give consent to a business to perform on the individual a tattooing procedure, and a parent, guardian or custodian of an individual under 16 years of age, but over 14 years of age who desires to give consent to a business to perform on the individual a body-piercing procedure, shall do both of the following:
(1) Appear in person at the business at the time the procedure is performed and remain through the procedure; and
(2) Sign a document provided by the business that explains the manner in which the procedure will be performed and the methods for proper care of the affected body area following performance of the procedure.
