§ 96.19 RECORDS TO BE MAINTAINED.
   Each franchise shall maintain all records required by law and will additionally maintain the following records:
   (A)   Record of dispatch. Shall show time call was received, time ambulance was dispatched, time arrived at scene, time arrived at destination, and time ready for service. Response time reports shall be available using the requirements set forth in the EMS Performance Improvement Center "EMS System Response" Toolkit.
   (B)   Trip record form. Shall be on a form or electronic data system approved by the county and compliant with the North Carolina College of Emergency Physicians Standards for EMS Data Collection and shall include all information required in division (A) of this section in addition to other information required by the county. The trip record shall be so designed as to provide the patient with a copy thereof containing all required information. A copy of the trip record may serve as a receipt for any charges paid.
   (C)   Daily report log. Shall be maintained for the purposes of identifying all persons/patients transported in any one 24-hour period.
   (D)   Daily driver and attendant checklist and ambulance inspection report. Shall list the contents and description of operations for each ambulance, signed by the individual verifying the ambulance's operations and that equipment is in clean and proper working order on a daily basis.
