§ 118.051 APPLICATION; FEE; BOND.
   (A)   Application for a license to operate a hotel or motel within the city shall be made to the City Clerk. The application shall be signed by the applicant as an individual, by the partners if a partnership, or by the officers and directors if a corporation or similar entity. The application shall contain the following information and statements:
      (1)   The name, age, and address of: the applicant, in the case of an individual; the persons entitled to share the profits, in the case of a partnership; officers and directors, in the case of a corporation or similar entity, as well as the date of incorporation/formation and the object for which it was incorporated/formed;
      (2)   The citizenship of the applicant, place of birth, and, if a naturalized citizen, the time and place of naturalization;
      (3)   The taxpayer identification number of the owner, and, if different, of each hotel or motel to be licensed;
      (4)   A statement as to whether a previous license issued by a state, municipality or federal government has ever been revoked or suspended, and the reasons for such revocation or suspension; and,
      (5)   A statement that the applicant will not violate any of the laws of this state, the United States of America, or any ordinance of the city in the conduct of his or her business.
   (B)   The applicant shall pay a license fee of $100 for each location where he or she intends on operating a hotel or motel.
   (C)   The applicant shall execute a bond, with good and sufficient surety, for each location where he or she intends on operating a hotel or motel. The amount of the bond shall be set at $100 per room contained within the hotel or motel. Once executed, the bond shall be filed in the office of the City Clerk, along with the application and license fee.
   (D)   Upon proper application and payment of the license fee, the Mayor shall issue, and the City Clerk shall countersign, the license for the operation of the hotel or motel.
