(A) Any person or organization desiring to conduct or operate any gun club for the discharge of firearms in the city shall make written application to the Mayor for a license to do so.
(B) The application shall contain the following information and statements:
(1) The name, age, and address of: the applicant, in the case of an individual; the persons entitled to share the profits, in the case of a partnership; officers and directors, in the case of a corporation or similar entity, as well as the date of incorporation/formation and the object for which it was incorporated/formed;
(2) The citizenship of the applicant, place of birth, and if a naturalized citizen the time and place of naturalization;
(3) The taxpayer identification number of the applicant;
(4) A statement as to whether a previous license issued by a state, municipality or federal government has ever been revoked or suspended, and the reasons for such revocation or suspension; and,
(5) A statement that the applicant will not violate any federal or state law or regulation, or any ordinance of the city in the conduct of their business.
