(A) Application for a license issued pursuant to this section shall be made to the City Clerk on forms provided by the city.
(B) The application shall contain the following information:
(1) The name, age, address and phone number of: the applicant, in the case of an individual; the persons entitled to share the profits, in the case of a partnership or similar entity; officers and directors, in the case of a corporation or similar entity as well as the date of incorporation and the object for which it was incorporated.
(2) The address of the proposed place of amusement and the type of amusement to be conducted.
(3) The taxpayer identification number of the applicant.
(4) A statement that the applicant has and shall maintain liability insurance with minimum coverage of $250,000 per occurrence and an aggregate of $500,000. Proof of such insurance coverage shall be filed with the application.
(5) A statement that the applicant shall execute a surety bond in the amount of $5,000 conditioned upon observance of all of the terms and provisions of the ordinances of the city and laws of the state.
(6) A statement that the applicant attests to the truth and accuracy of the information contained in the application.
