§ 110.03 APPLICATION; FEE.
   (A)   Each person who wishes to canvass, solicit or peddle within the village, including employees of any corporation, limited liability company, partnership or any other entity shall apply for and receive a permit prior to engaging in any canvassing, solicitation or peddling within the village.
   (B)   Applicants for a permit under this chapter must file with the Mayor a sworn application in writing (in duplicate) on a form to be furnished by the Mayor which shall give the following information:
      (1)   Name of the applicant;
      (2)   Permanent home address and full address of the applicant;
      (3)   The name, address and telephone number of each person or entity for whom or upon whose behalf the applicant will be canvassing, soliciting or peddling within the village;
      (4)   A brief description of the nature of the business and the goods to be sold;
      (5)   The length of time for which the right to do business is desired;
      (6)   The place where the goods or property to be sold, or orders taken for the sale thereof, are manufactured or produced, where the goods or products are located at the time the application is filed, and the proposed method of delivery;
      (7)   The make and model and license number of the automotive vehicle or vehicles the applicants will be using to conduct business;
      (8)   If the applicant is a professional solicitor, as defined in R.C. § 1716.01, the applicant must provide a copy of its current registration with the Attorney General of the state.
   (C)   At the time of filing the application, a fee of not more than $25 shall be paid to the Mayor to cover the cost of the permit. The permit shall expire 180 days from the date of issuance.
