The owner of a dog or cat for which registration is required shall provide proof of current vaccination against rabies, and shall complete an application for registration, which shall be in writing on blanks provided by the City Clerk, and shall state the breed, sex, age, color, and name of the dog or cat; and the address, telephone number(s), and signature of the owner. The application shall also state the date of the most recent rabies vaccination and the date for revaccination. The owner shall state whether a dog being registered is a “high risk dog” as defined in Section 55.01of this Code of Ordinances.
