A person desiring to conduct a private motor vehicle escort agency or business in the city shall file with the chief of police a written application on a form provided for the purpose, duly signed and verified, which shall state the full name, age, and residence of the licensee, shall specify the trade name and address or location from which the business or agency will operate and for which the escort license is desired, and other relevant facts as may be required by the chief of police.
