§ 94.21 CONTENT OF APPLICATION.
   The application for a permit shall set forth the following information:
   (A)   The name, address, and telephone numbers, daytime and nighttime, of the natural person who will be responsible for performance of the duties of the permit holder;
   (B)   The date and time when the event is to be conducted;
   (C)   Proposed location of the event, including a map of the proposed area to be used which shows any barricades, street route plans (including the starting point, the termination point, and the desired location of any assembly areas), or perimeter/security fencing;
   (D)   Estimated numbers of event staff, participants, and spectators;
   (E)   Any public health plans, including supplying water to the site, solid waste collection, and provision of toilet facilities;
   (F)   Any fire prevention and emergency medical service plans, including services to be provided by city personnel;
   (G)   Any security plans, including services to be provided by city personnel;
   (H)   Any special services to be provided by city employees;
   (I)   Whether food or alcohol is to be served or sold at the event, including all licensing information for food or alcohol vendors, such licensing information to be provided at least 14 days before the event;
   (J)   Signature of the applicant;
   (K)   If the event is an event planned within a residential area in which a street is to be closed, the applicant must collect and submit with the application a list of signatures from all properties whose vehicular access to their properties will be affected consenting to the street closure; and
   (L)   Such other information as the Clerk-Treasurer shall find necessary to evaluate the application.
