(A) A person making a sale or transfer of possession of model glue to a minor who exhibits and delivers the written consent of his or her parent or guardian shall record the name, address, sex and age of the minor and the name and address of the consenting parent or guardian.
(B) All data required by this section, including the written consent, shall be kept in a permanent register available for inspection by the Police Department for a period of at least six months immediately following such sale or transfer.
