Any protest shall be in writing and shall include the following information:
(A) The name, current address, and telephone number of the protester;
(B) The signature of the protester or its representative;
(C) Identification of the solicitation or contract number; and
(D) A detailed statement of the legal and factual grounds of the protest, including:
(1) Copies of relevant documents; and
(2) The form of relief requested.
