Applications for excavation permits shall be made to the director and shall include the following:
(1) Location of the intended excavation;
(2) The size of the intended excavation;
(3) The purpose of the intended excavation;
(4) The person, firm, corporation, association, or others doing the actual excavation;
(5) The name of the person, firm, corporation, association, or others for whom the work is being done;
(6) Signed agreement that the applicant will comply with all ordinances and laws relating to the work to be done;
(7) Specific time frames for the completion of the excavation and for the completion of the restoration.
Such application shall be rejected or approved by the director within twenty-four (24) hours of its filing.
