A permit shall be obtained from the department prior to beginning the installation of a water well or use of a spring in the City of Cookeville. If an application is approved by the department, the applicant shall be issued a permit. Receipt of the permit shall constitute permission to begin construction. Such permits may be revoked by the department upon the violation by the holder of any terms of the permit or these rules and regulations or in any emergency when, in the judgment of the department, the continued operation of the water supply for any reason shall constitute a health hazard. The holder of such permit, after such revocation, shall have the right of appeal
