Each license issued under the provisions of this chapter shall state upon its face the following:
A. Name of the licensee and any other name under which business is to be carried on.
B. Number of employees as herein defined.
C. The local address and phone number of the business.
D. The amount of the license fee.
E. The date of issuance and date of expiration.
F. If a public utility, the number of connections within the city.
G. Such other and further information as the ordinances of the city shall require. (1975 Code § 3-1-11; amd. 2015 Code)
