It shall be the duty of every person to apply for a license to the Humane Officer or to the authority deemed appropriate by the Animal Control Commission. The application, one per animal, shall include the name and address of the applicant, a description of the animal, and a current rabies certificate issued by a veterinarian. If applicant withholds or falsifies any information of the application, no license shall be issued, and any licenses issued upon false or withheld information shall be null and void. Application for a license must be made when the animal reaches the age of six months. When a person obtains an animal older than six months, a license must be applied for within 14 days.
