(A) An application to sell, give, transfer, or otherwise dispose of any item, effect, paraphernalia, accessory, or thing which is designed or marketed for use with illegal cannabis or drugs shall be made to the Clerk-Treasurer of the city and shall be on the form prescribed by the Clerk-Treasurer.
(B) The application to sell, give, transfer, or otherwise dispose of any item, effect, paraphernalia, accessory, or thing which is designed or marketed for use with illegal cannabis or drugs shall be accompanied by appropriate affidavits of the applicant and of each and every employee authorized to sell, give, transfer, or otherwise dispose of the items that such person has never been convicted of a drug-related offense.
(C) The application shall be accompanied by an annual licensing fee of $1,000.
