§ 153.49 FINAL PLATS FOR ALL SUBDIVISIONS.
   (A)   The final plats shall depict or contain the following information.
   (B)   Final plats shall be clearly and legibly drawn by a registered land surveyor currently licensed in the state by the State Board of Registration for Professional Engineers and Land Surveyors.
   (C)   The plat shall also be drawn at a scale of not less than 200 feet to one inch and shall be drawn on a sheet size of Mylar acceptable to the Register of Deeds of the county.
      (1)   Final plat requirements for all subdivisions.
         (a)   Labeling:
            1.   Name and address of owner of record;
            2.   Name of subdivision;
            3.   Location (town, county, state);
            4.   Date(s) of survey(s);
            5.   Graphic scale and written scale (common engineering scale between one inch equals 20 feet and one inch equals 200 feet);
            6.   North arrow;
            7.   Name, address, registration number and seal of registered land surveyor;
            8.   Tax parcel identification number;
            9.   Vicinity map showing the relationship between the proposed subdivision and surrounding area;
            10.   Boundaries of the tract shown with bearings and distances;
            11.   Names of adjacent property owners;
            12.   Lot lines and corners, lot numbers, bearings and distances and other dimensions including acreage;
            13.   Areas reserved for school sites, parks or other open spaces;
            14.   Location of the flood hazard, floodway and flood fringe boundaries, if applicable;
            15.   Existing buildings or structures, railroads and bridges on the land to be subdivided; and
            16.   Names of adjacent property owners.
         (b)   Roads:
            1.   Existing and platted roads location and dimensions; and
            2.   Road name(s).
         (c)   Utilities:
            1.   Utility and other easements of record on and abutting to the subdivision;
            2.   Transmission lines;
            3.   Approximate location of natural gas lines;
            4.   Sanitary sewers, location and size if community systems are proposed; and
            5.   Storm sewers, culverts, detention ponds and other drainage facilities, if any.
         (d)   Site calculations:
            1.   Acreage in total tract to be subdivided;
            2.   Total number of lots;
            3.   Linear feet in roads, if applicable;
            4.   Area of each lot in acres; and
            5.   Minimum setbacks in table and graphic form.
         (e)   Other details:
            1.   Location of control corners;
            2.   Location and description of all monuments, reference markers and property and lot corners designation of any and all wetlands;
            3.   For family subdivisions, a road maintenance agreement signed by all affected parties shall be filed with the final plat;
            4.   A subdivision roads disclosure statement prepared in accordance with G.S. § 136-102.6(f) below:
Prior to entering any agreement or any conveyance with any prospective buyer, the developer and seller shall prepare and sign, and the buyer of the subject real estate shall receive and sign an acknowledgment of receipt of a separate instrument known as the subdivision streets disclosure statement (hereinafter referred to as disclosure statement). The disclosure statement shall fully and completely disclose the status (whether public or private) of the street upon which the house or lot fronts. If the street is designated by the developer and seller as a public street, the developer and seller shall certify that the right-of-way and design of the street has been approved by the Division of Highways, and that the street has been or will be constructed by the developer and seller in accordance with the standards for subdivision streets adopted by the Board of Transportation for acceptance on the highway system. If the street is designated by the developer and seller as a private street, the developer and seller shall include in the disclosure statement an explanation of the consequences and responsibility as to maintenance of a private street, and shall fully and accurately disclose the party or parties upon whom responsibility for construction and maintenance of the street or streets shall rest, and shall further disclose that the street or streets will not be constructed to minimum standards, sufficient to allow their inclusion on the state highway system for maintenance. The disclosure statement shall contain a duplicate original which shall be given to the buyer. Written acknowledgment of receipt of the disclosure statement by the buyer shall be conclusive proof of the delivery thereof
            5.   Any other information considered by the subdivider and/or Subdivision Administrator to be pertinent to the review of the plat.
         (f)   Certificates for all plats:
            1.   Certificate of ownership and dedication:
   I hereby certify that I am (we are) the owner(s) of the property shown and described hereon, and that I (we) hereby adopt this plan of subdivision with my (our) free consent, and dedicate all roads and other sites and easements to public use as noted in the Disclosure of Private Roadways, where applicable.

   If the Health Department has not approved private water or septic systems then the following written statement shall be included on the plat:
"The District Health Department has expressed no opinion as to the suitability of private septic or water systems on this property. Each lot is subject to individual inspection and approval of septic systems."
         (g)   Certificates for major subdivision final plats only:
            1.   Certificate of approval of design and installation of streets, utilities and other required improvements (if applicable):
   I hereby certify that all streets, public utilities and other required improvements have been installed in an acceptable manner and according to N.C. Department of Transportation and/or Town of Columbus specifications and standards or as otherwise provided for in this chapter, or that guarantees for the installation of the required improvements in an amount and manner satisfactory to the Town of Columbus have been submitted.

            2.   Private road maintenance statement (if applicable): a maintenance statement for private roads shall be submitted to the Planning Board with the final plat and recorded at the Register of Deeds at the time the plat is recorded.
      (2)   Two-lot, family and special subdivision final plat submittal requirements:
         (a)   Two full-size paper copies and one Mylar copy for signature; and
         (b)   One full-size paper copy of the recorded plat.
      (3)   Minor and major subdivision final plat submittal requirements:
         (a)   Two full-size paper copies and one Mylar copy for signature;
         (b)   One full-sized paper copy of the recorded plat; and
         (c)   One digital copy in .pdf and .dwg format.
