§ 153.48  PRELIMINARY PLATS FOR MINOR AND MAJOR SUBDIVISIONS.
   (A)   The preliminary plat shall depict or contain the following information.
   (B)   Where "proposed/actual" is noted, the proposed information shall be shown on the preliminary plat and the actual information shall be shown on the final plat. Preliminary plats shall be clearly and legibly drawn at a scale of not less than 200 to one inch acceptable to the Register of Deeds of the county.
      (1)   Labeling:
         (a)   Name and address of owner of record;
         (b)   Name of subdivision;
         (c)   Location (town, county, state);
         (d)   Date(s) of survey(s);
         (e)   Graphic scale and written scale (common engineering scale between one inch equals 20 feet and one inch equals 200 feet);
         (f)   North arrow;
         (g)   Name, address and registration number of registered land surveyor;
         (h)   Tax parcel identification number;
         (i)   Vicinity map showing the relationship between the proposed subdivision and surrounding area;
         (j)   Boundaries of the tract shown with bearings and distances;
         (k)   Map depicting the proposed phases of the subdivision and lots and/or sections contained therein and a tentative phasing schedule/timeline;
         (l)   Names of adjacent property owners;
         (m)   Proposed lot lines and corners, lot and block numbers, and approximate dimensions including acreage;
         (n)   Existing buildings or structures, railroads and bridges on the land to be subdivided;
         (o)   Existing uses of the land within the proposed subdivision;
         (p)   Location of any cemetery;
         (q)   Proposed school sites, parks or other open spaces; and
         (r)   Location of the flood hazard, floodway and flood fringe boundaries, if applicable.
      (2)   Roads:
         (a)   Existing and platted roads on abutting properties and in the proposed subdivision with rights-of-way, location and dimensions;
         (b)   Road name(s) and designation of private or public dedication;
         (c)   Linear feet in roads;
         (d)   Street cross sections showing right-of-way-width, pavement width, and shoulder, as well as any additional features such as curb and gutter, sidewalks, street trees and street lights; and
         (e)   State Department of Transportation (NCDOT) written approval of any public road plans, if applicable.
      (3)   Utilities:
         (a)   Utility and other right-of-ways or easements of record on and abutting to the subdivision;
         (b)   Transmission lines;
         (c)   Approximate location of natural gas lines;
         (d)   Approximate location of water and sanitary sewers existing, approximate location, size and plans for connection to private community or public community or public systems; and
         (e)   Storm sewers, culverts, detention ponds and other drainage facilities, if any (proposed/actual).
      (4)   Site calculations:
         (a)   Acreage in total tract to be subdivided;
         (b)   Total number of lots and/or units;
         (c)   Area of each lot in acres;
         (d)   Acreage in open space and percentage of total development area;
         (e)   Acreage in parks, lakes or other public land usage;
         (f)   Average lot size; and
         (g)   Proposed minimum building setback lines or note explaining setback lines.
      (5)   Other details:
         (a)   Location and name of all water courses, as identified on U.S.G.S. seven and one-half minute series (topographic) maps;
         (b)   Note all lots or building sites with an elevation of 3,000 feet above sea level or greater;
         (c)   Stormwater plan and analysis;
         (d)   Environmental survey for major subdivisions in accordance with § 153.62(B) showing existing conditions including topography, slopes greater than 50%, large stands of trees, any tree over 18 inches in diameter, significant rock outcroppings, slopes steeper than 15%, floodplains, floodways, wetlands, perennial streams, natural drainageways, lakes, other water bodies and the presence of any known endangered species indicated in any surveys completed by the state or other governmental agency. Photographs representative of the property's features shall also be provided;
         (e)   Landscaping plan, if applicable;
         (f)   Lighting plan, if applicable; and
         (g)   Other infrastructure details deemed necessary by the Subdivision Administrator.
      (6)   Certificates and other documentation:
         (a)   Certificate of approval for preliminary plat for minor subdivisions.
   This certifies that the Town Planning Board approved the preliminary plat of the ______________ Subdivision on the ___ day of ______________, 20___.

         (c)   Certificate of steep terrain health protection (if applicable).
   I hereby certify that all lots on slopes of greater than 20% meet the provisions and minimum lot size requirements as set forth in Section 5.3.3 of the Town of Columbus Subdivision Ordinance.

         (d)   Grading and Erosion Control Plan and a North Carolina Department of Environment Natural Resources (NCDENR) written approval, if applicable.
         (e)   Written approval from the town for provision of public water and sewer, if applicable.
         (f)   NCDENR written approval for a community or public sanitary sewer system, if applicable.
         (g)   NCDENR written approval for a public or community water system, if applicable.
         (h)   A written statement, if applicable, that: "The Polk County Health Department has expressed no opinion as to the suitability of private septic systems or water systems on this property. Each lot is subject to individual inspection and approval of septic systems."
         (i)   A copy of any required NCDOT driveway permit application.
         (j)   Any other information considered by the subdivider, the Planning Board, the Town Council and/or Subdivision Administrator to be pertinent to the review of the plat.
   (C)   Preliminary plat submittal requirements:
      (1)   Three full-size copies for staff review;
      (2)   Four full-size copies and one reduced (11 inches by 17 inches) copy for Planning Board and Town Council; and
      (3)   One digital copy in .pdf and .dwg formats.
