A. A significant industrial user shall apply for wastewater discharge permit re-issuance by submitting a complete wastewater discharge permit application a minimum of thirty days prior to the expiration of the industrial user's existing waste discharge permit.
B. When requested, the control authority may conduct the sampling and monitoring for the industrial user as required in the industrial discharge permit. The industrial user may perform sampling and monitoring "in-house" or by a contract laboratory. However, if sampling by an industrial user indicates a violation, the industrial user, upon becoming aware of a violation, must notify the control authority of the violation within twenty-four hours, then repeat the analysis and submit the results within thirty days.
C. Industrial users must notify the control authority immediately of all discharges that could cause problems to the POTW, including any slug loads.
D. The industrial user must promptly notify the control authority in advance of substantial changes in volume or character of discharges, including changes in any listed or characteristic RCRA hazardous wastes discharged to the POTW.
